Female Dr. has ALS, returns to hometown, meets highschool sweetheart

Can anyone tell me the name of this novel???  Several years ago I read a very moving novel about a female physician, who was married with a teenage daughter.  She discovered she had early symptoms of ALS.  Either her father was ill, or her father had died and she had to return to her hometown to sell his farm?  Or was she in a friend's wedding?  She bumps into her high school sweetheart in the small town, who is a rock star.... without too many spoilers, can anyone help me with the title of this book???  It's one I really enjoyed.
